Job Description

I need a Jazz MUSIC Template Designed by a professional and the person must show previous work in this area. I have listed some Site which gives you some ideas of what I am looking for. Also, I need the template bigger so I can run adsense videos on the homepage and their smallest player is 400x415pix. The designer MUST HAVE: Great communication skill. I need a completion date and or target date of completion. I need screenshots of my template for approval. I need support after delivery for possible installation and debugging issue in other words you must support you product.
