Advanced Polling

I am looking for a mod to Dolphin 6.1.1 existing polling capabilities, or an add-on if that becomes easier.

Advanced Polling features should include:
- Ajax or Flash based, look & feel of site
- Poll title, questions, and multiple answer formats (radio, checkbox, text box)
- Poll scheduling w/start and end dates
- Ability for users to create polls. Admins should also be able to create polls on behalf of the site or certain users.
- Ability for subscribed users to request poll placement on home page or other site pages. Admin/moderator approval and scheduling required. Ability to tie into payment system.
- By default, all user-generated polls displayed on "Polls" page and their profile, in addition to above requested locations.
- Track users who have responded (their user id and response).
- Users may only vote once per poll.
